Job summary

A healthcare services provider is hiring experienced civilian Social Workers to support the Exceptional Family Member Program at Cannon Air Force Base in New Mexico. Responsibilities include coordinating care and support for military families with special needs. Applicants must have a Master's degree in Social Work and relevant experience. Qualifications

• Minimum of two years full-time experience in a medical setting in care coordination and case management.
• Current Basic Life Support (BLS) certification required.

Responsibilities

• Oversee and manage the EFMP-M program in alignment with DoD and Air Force policy.
• Identify and support families with special medical or educational needs.
• Ensure accurate enrollment and maintain data in special needs systems.
• Train personnel and provide consultation on EFMP-M implementation.
