Job summary

Virginia Commonwealth University Health Systems Emergency Department is seeking an Emergency Department Clinical Social Worker 2 WIP to deliver discharge planning and social work services during normal business hours and after hours, working with neonatal through geriatric patient populations.

• Master's of Social Work from a CSWE-accredited program.
• Minimum of one year of clinical social work experience.
• Preferred experience in acute care discharge planning and related settings.

Responsibilities

• Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients in the Emergency Department.
• Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost-effective discharge plans.
