Responsibilities

• Provide comprehensive psychosocial assessment for identified patients and families.
• Foster interpersonal relationships with patients, families, physicians, staff and community partners.
• Mentor junior staff and students; educate on psychosocial topics affecting care and family dynamics.
• Respond to trauma activations, medical alerts, codes and provide crisis intervention.
• Participate in discharge planning and ensure continuity of care across teams.
