Qualifications

• Master of Social Work from an accredited institution.
• Current state licensure (LCSW) obtained prior to or on supervision hours.
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Driver’s license and reliable transportation.

Responsibilities

• Assess and screen patients for transition planning, mental health and goals of care.
• Develop and monitor care plans addressing social determinants and resources.
• Provide brief psychotherapy and psychosocial assessments as indicated.
• Identify disorders and connect patients to services and supports.
• Educate, advocate, and facilitate safe transitions of care to the community.
• Collaborate with care teams, insurers, and community providers.
