Job summary

Intermountain Health is seeking a Social Work Care Manager I in Las Vegas, NV to perform psychosocial assessments, develop care plans, and provide clinical counseling with a collaborative care team.

The role requires an MSW and LCSW licensure, strong communication, and the ability to coordinate care across settings. It is a full-time, dayshift position with a comprehensive benefits package.

Qualifications

• MSW from an accredited institution (degree verification required).
• Current LCSW licensure, or applicable licensure obtained prior to/completion of supervision hours.
• Basic computer proficiency (word processing/spreadsheet).
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking.

Responsibilities

• Assess patients for transition planning, mental health, substance use, and goals of care.
• Develop and monitor care plans addressing social determinants and resources.
• Provide motivational interviewing and therapeutic techniques for behavioral health.
• Provide brief individual, group, and family therapy and psychosocial assessments.
• Identify mental, emotional, and behavioral disorders and connect patients to services.
• Educate staff, advocate for rights and access to care, and facilitate safe transitions of care.
• Collaborate with healthcare teams, insurers, and community providers for quality care.
• Lead improvement initiatives and ensure policy adherence.
• Facilitate clinical goals of care discussions with patients and families.
