Qualifications

Responsibilities

• Identify patients for proactive intervention using payor models and health-risk criteria.
• Coordinate care to ensure continuity and safe transitions between care settings.
• Assess functioning, resources, psychosocial needs, and support systems.
• Collaborate with care team, patients, and families to set goals and actions.
• Coordinate discharge planning and home care per guidelines and payor models.
• Educate patients and caregivers on health management and resources.
• Participate in care rounds, quality improvement, and payor communications.
