Independence Health System is seeking an Emergency Department Pharmacist to lead the Pharmacist-Led Medication Reconciliation initiative and support ED clinical services across rounds and non-rounding activities.

The role emphasizes collaboration with Pharmacy Managers, ED physicians, nurses, informaticists, and IT staff to ensure safe, cost-conscious medication therapy and regulatory compliance with The Joint Commission, USP, and state guidelines.

Qualifications

• Graduate of an accredited school of pharmacy with a BS or Pharm.D.
• Three years’ experience in an acute care setting or completion of a PGY1 Residency.
• Strong leadership, organizational skills, independent and critical thinking, sound judgment, and knowledge of legal aspects of pharmacy practice.

Responsabilités

• Develop and review evidence-based guidelines and protocols for medication reconciliation.
• Participate in collaborative care rounds when needed.
• Assure medication safety and report medication errors and adverse drug reactions.
• Oversee pharmacokinetic monitoring and performance improvements.
• Serve as liaison between staff pharmacists, leadership, and clinical staff.
