Job summary

11th Hour Service is seeking a Clinical Staff Pharmacist – Emergency Medicine in Winchester, VA for a hybrid role combining operational staffing with advanced clinical responsibilities in a high-acuity ED.

The pharmacist will verify orders, provide therapeutic consultation, develop ED policies, and lead medication-safety initiatives while collaborating with physicians, nurses, and leadership to optimize patient outcomes.

Qualifications

• PharmD from ACPE-accredited program is required.
• Successful completion of PGY-1 residency is required.
• PGY-2 residency in emergency medicine or critical care is preferred.

Responsibilities

• Perform real-time order verification and therapeutic consultation in the ED.
• Develop and optimize ED-specific policies, order sets, and clinical pathways.
• Promote formulary compliance and lead performance improvement initiatives for medication safety and patient outcomes.
• Identify and resolve medication-related issues and participate in emergency response and critical care decisions.
• Document clinical interventions and cost avoidance per organizational standards.
• Collaborate with physicians, nurses, and leadership to address safety concerns and reduce error-prone processes.
• Contribute to medication-use policy development and support P&T activities.
• Engage in staff education, competency development, and precepting pharmacy learners.
