Job summary

Hudson Regional Health, part of the CarePoint Health network, seeks a Patient Navigator to support hepatitis C, B, and HIV testing in the emergency department. The role uses USPSTF criteria to identify candidates and facilitates testing and follow-up within our hospital system.

Education includes an associate degree in healthcare or public health, or Medical Assistant with relevant experience. Strong communication and bilingual abilities are preferred for diverse patient outreach.

Qualifications

• Associate degree in healthcare or public health required or Medical Assistant plus 2 years of clinic operations experience.
• Knowledge of HIV and Hepatitis disease fundamentals a plus.
• Experience with EHR systems and clinical workflows.

Responsibilities

• Assist ED staff in identifying patients meeting USPSTF criteria for hepatitis C, hepatitis B, and HIV testing.
• Order testing for eligible patients and verify country of birth for Hoboken ED patients.
• Collaborate with clinic services to screen patients with advanced liver damage via FIB-4 scores.
• Provide disclosure of positive results and linkage to infectious disease or liver clinics.
