Job summary

SUNY Upstate Medical University is seeking an RN Navigator to manage medications for patients with chronic illnesses. The role monitors progress and collaborates with a multidisciplinary team to ensure timely screening and optimal care across the health care continuum.

You will educate patients about their disease and treatment, maintain a professional care environment, and complete new patient intakes for HIV, Hep C, and PEP, ensuring follow-up and appropriate referrals when needed.

Qualifications

• NYS licensed/registered RN or eligible
• Bachelor's degree in Nursing or healthcare-related field with 3 years RN experience or Associate's degree with 5 years RN experience
• Excellent written and oral communication and interpersonal skills
• CPR-BLS certification from AHA / Red Cross required by appointment

Responsibilities

• Provide medication management and monitor patient progress
• Collaborate with multidisciplinary team to ensure timely screening, intervention and care
• Educate patients about disease and treatment across the care continuum
• Complete new patient intakes for HIV, Hep C and PEP patients and ensure follow-up
• Facilitate referral to higher levels of case management when needed
