Job summary

Qualifications

• Minimum Education and Experience Requirements: Pharmacy Technician Level 1; 12 months or 6 months Shands Pharmacy Technician experience with additional criteria; ability to work independently; ability to contribute to team relations; experience managing projects preferred; Microsoft Office Suite preferred.

Responsibilities

• Obtain and document comprehensive medication histories from patients, caregivers, and external sources.
• Document home medications with drug name, dose, route, frequency, and last dose.
• Identify discrepancies and potential issues for pharmacist review.
• Prioritize high-risk patients for timely reconciliation in the ED.
• Interview patients/caregivers clearly, including those with limited health literacy.
