Job summary

UF Health in Gainesville, FL is seeking a dedicated Medication History Pharmacy Technician to support safe medication reconciliation in a fast-paced ED environment. You will interview patients, caregivers, and external sources, and document home medications with dose, route, frequency, and last dose taken.

The role requires adherence to HIPAA, attention to detail, and collaboration with pharmacists, providers, and nurses to optimize patient safety and outcomes.

Qualifications

• Must meet all requirements of a Pharmacy Technician Level 1, including required competencies.
• 12 months of Shands Pharmacy Technician experience.
• 6 months of Shands Pharmacy Technician experience and one of: two years hospital/institutional or retail pharmacy experience.
• Demonstrate the ability to work independently.
• Demonstrate the ability to contribute to effective working relationships.
• Experience managing projects preferred.
• Experience with Microsoft Office Suite preferred.

Responsibilities

• Conduct comprehensive medication history interviews with patients, caregivers, and external sources.
• Accurately document home medications, including drug name, dose, route, frequency, and last dose taken.
• Identify and clarify discrepancies, duplications, and potential medication-related issues for pharmacist review.
• Prioritize high-risk patients to ensure timely medication reconciliation in the ED.
• Interview patients and caregivers using effective communication techniques, including acutely ill or limited health literacy.
• Provide clear explanations of the medication history process and its importance to patient safety.
• Escalate clinical concerns or complex medication issues to the pharmacist.
• Ensure compliance with regulatory standards, patient privacy (HIPAA), and institutional guidelines.
• Maintain thorough and timely documentation to support medication reconciliation workflows.
