Job summary

Veterans in Healthcare in State College, PA, seeks an ED Case Manager to support patient flow and care coordination on-site. You will evaluate patients, perform InterQual assessments, and guide discharge planning for appropriate levels of care.

Responsibilities include assessing behavioral health patients, collaborating with ED physicians on safety and status, and communicating with families and care teams to achieve optimal outcomes. Evening/night shifts and weekend coverage are required.

Qualifications

• Bachelor’s degree in a related field; a BSW or Psychology degree preferred.
• Associate degree in Social Work or Psychology with 8 years acute care experience or 4 years care manager experience considered.
• Minimum of two years health care experience, preferably in acute care.
• Preference for psychiatric, utilization management, and care protocols experience.

Responsibilities

• Provide guidance on patient placement and level of care in the ED.
• Complete initial InterQual assessment and assist with discharge planning.
• Assess behavioral health patients and coordinate with physicians on safety and status.
• Communicate with families and care team to ensure best patient outcomes.
