Job summary

Tampa General Hospital is seeking a Case Manager, RN for ED Care Coordination to ensure safe, efficient patient transitions and ongoing care planning. You will work with physicians and the interdisciplinary team to set LOS estimates, discharge goals, and post-acute placement while addressing barriers to care.

You will lead and participate in rounds, update PSAPs for frequent ED utilizers, and coordinate with social work for psychosocial needs to reduce readmissions.

Qualifications

• Registered nurse with Florida license is required.
• Five years nursing experience with at least two in Case Management or two in Emergency Medicine.
• Graduate of an accredited School of Nursing; Associates Degree required.

Responsibilities

• Coordinate discharge planning and transition plans across care levels.
• Collaborate with physicians and interdisciplinary team to determine care plan and discharge disposition.
• Lead or participate in care coordination rounds and huddles to monitor patient progress.
• Identify barriers to timely discharge and communicate needs to the team.
• Develop and update Patient Specific Action Plans for high utilizers in ED.
• Ensure accurate data entry and documentation across information systems.
