Job summary

Qualifications

• BSN preferred; nursing degree considered.
• At least 2 years of clinical nursing experience.
• Knowledge of disease management, reimbursement, and levels of care.

Responsibilities

• Coordinate patient-centered care for individuals with acute and chronic conditions.
• Facilitate interdisciplinary collaboration across care teams.
• Ensure smooth transitions across care settings.
• Serve as an expert resource on care protocols, resource utilization, and regulatory standards.
• Partner with patients and families to identify healthcare issues, address barriers to self-care, and provide culturally sensitive education.
• Utilize quality improvement methods to monitor interventions and ensure fiscally responsible care coordination across the continuum.
