Job summary

Access TLC is seeking an experienced ECM/CS Program Coordinator to lead the day-to-day operations of the Enhanced Care Management and Community Supports programs at our Moorpark office, with regular travel to the San Fernando office to support the team.

You will supervise Lead Care Managers, manage referrals and enrollment workflows, ensure compliance and quality standards, and drive continuous improvement across the program.

Qualifications

• Minimum 2 years in ECM/Medi-Cal/related care coordination program.
• Familiarity with CalAIM and ECM/CS Programs.
• Prior supervisory or team lead experience preferred.
• Bachelor's or Master's degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, Health or Human Services.
• California Driver's License with vehicle and insurance.
• Fluent in Spanish and English (preferred).
• Strong organizational, communication, documentation, and leadership skills.
• Ability to manage multiple priorities and meet deadlines with Managed Care Plans.

Responsibilities

• Supervise Lead Care Managers, providing guidance and performance oversight.
• Oversee day-to-day ECM and CS operations to ensure performance and compliance.
• Monitor caseload distribution, capacity, and workflow across the care management team.
• Manage referral, enrollment, and disenrollment processes across contracted Managed Care Plans.
• Monitor Community Supports referrals and delivery of authorized services.
• Coordinate with Managed Care Plan representatives on requirements, reporting, and issue resolution.
• Support implementation and adherence to program policies and procedures.
• Track program KPIs and prepare required reports and data submissions.
• Conduct quality assurance reviews of care management documentation.
• Support audit preparation and respond to audit requests.
• Support onboarding and ongoing training of team members.
• May carry a small caseload to support team capacity.
• Travel to assigned locations; mileage reimbursed.
