Job summary

Qualifications

• Position subject to Child Abuse, Criminal History, and FBI clearances for employment.
• Onsite in-person attendance is required as directed by Supervisor or Director.
• Pennsylvania residency is not a requirement for this position.

Responsibilities

• Screen all referrals to determine resources, priorities and concerns, and identify supports and services necessary to enhance the family’s capacity to meet the child’s developmental needs.
• Track at-risk children who do not meet the eligibility for Early Intervention Services but meet criteria for tracking services.
• Coordinate the Multi-Disciplinary Evaluation within 45 days of initial referral and facilitate the IFSP and transition plan.
• Follow up on health, safety or other concerns identified at the time of the visit.
• Document activities of monitoring in case comments and billed to MA with 60% productivity for billable activities.
• Assist families in gaining access to early intervention services and other services in the child’s natural environment.
• Attend at least 24 hours of training annually, in addition to pre-service training relevant to early intervention or related fields.
• Other duties as delegated.

Job description

The Early Intervention Service Coordinator is responsible for effectively and efficiently managing a caseload and assisting each eligible family in identifying, locating, coordinating and monitoring mandated services for children birth to 3 years of age.

• Initial screening is completed for all referrals made for Early Intervention Services to determine resources, priorities and concerns of the family and to identify the supports and services necessary to enhance the family’s capacity to meet the developmental needs of the child.
• Tracking at-risk children who do not meet the eligibility for Early Intervention Services, but who meet the criteria for tracking services
• The Service Coordinator is responsible to coordinate the Multi-Disciplinary Evaluation within 45 days of initial referral and facilitating and participating in the development, implementation, annual review and evaluation of the Individual Family Service Plan (IFSP), and transition plan.
• The Service Coordinator is responsible for following up on any health, safety or other concerns identified at the time of the visit.
• Service Coordinator documents activities of monitoring in case comments and billed to MA, demonstrating productivity of 60% for all billable activities.
• The Service Coordinator is responsible for assisting the family of an infant or toddler with a disability in gaining access to the early intervention services and other services, in the child’s natural environment, as identified on the Individual Family Service Plan, including assisting the family in arranging to receive medical and health services and providing the family with appropriate community resources.
• Attends at least 24 hours of training annually, in addition to any pre-service training relevant to early intervention services, child development, community resources or services for children with disabilities.
• Other duties as delegated
• Bachelor’s degree which includes 12 college credits in early intervention, early childhood, special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences and
• 1 year of full-time or full-time equivalent experience working with or providing counseling to children, families or individuals with disabilities.
