Job Location: Carbondale, PA 18407-2304
Salary Range: $19.50 - $21.90 hourly

Responsibilities
  • Responsible for service coordination services to an assigned caseload of children birth to 3 years of age.
  • Maintain flexible work schedule that includes evenings and weekends to meet the needs of children and their families.
  • Coordinating initial and ongoing evaluations and assessments of children and their families.
  • Responsible for screening utilizing the Ages and Stages screening tool and tracking infants and toddlers on a caseload who are eligible for the at risk tracking program (Watch As I Grow).
  • Responsible for facilitating and participating in the development, implementation, review and monitoring of the Individualized Family Service Plan (IFSP).
  • Responsible to assist with identifying and gaining access to EI services and other community supports identified on the Individualized Family Service Plan (IFSP).
  • Is responsible to facilitate the timely delivery of EI services.
  • Responsible for and/or assisting with identifying available service providers by sharing with families Statewide ITF Provider List and L/S Provider list and facilitating communication with and between the family/caregiver and service provider.
  • Responsible for coordinating and monitoring of the delivery of EI services.
  • Responsible to inform families/caregivers of their parental rights agreement and available procedural safeguards, as well as the availability of advocacy services.
  • Assisting in the arrangement of medical and/or other health services, an infant/toddler needs or is receiving.
  • Responsible for collecting Early Childhood Outcome (ECO) data from providers at 5 months and 11 months of starting services and monitoring child’s progress.
  • Offering families/caregivers opportunities and support to participate in community activities and events with other infants/toddlers and their families/caregivers.
  • Facilitating the development of the transition plan as part of the IFSP and working collaborating with the Preschool EI Program.

This service is delivered exclusively in the field (pre‑pandemic). A valid PA driver’s license and use of a vehicle is essential.

Qualifications
  • A bachelor’s degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences, and 1 year of full‑time or full‑time‑equivalent experience working with or providing counseling to children, families or individuals with disabilities.
  • An associate’s degree, or 60 credit hours, from an accredited college or university in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology, or other comparable social sciences, and 3 years of full‑time or full‑time‑equivalent experience working with or providing counseling to children, families or individuals with disabilities.
  • PA Act 33 Child Abuse, PA Act 34 Criminal Background Check, and FBI Criminal Background Check clearances.
  • Ability to communicate effectively with professionals, children, and families.
  • Demonstrates the ability to maintain a positive and professional demeanor in working with children and their families.
  • Embrace a team approach by assisting staff willingly.
  • Evidence of good interpersonal skills to deal effectively with children and their families and Friendship House team.
  • Evidence of good written and communication skills and the ability to read, analyze and interpret written correspondence.
  • Evidence of computer capabilities necessary to utilize Pelican, the statewide Early Intervention database along with CareLogic, the Friendship House electronic medical record.
  • Exhibit a self‑directed and disciplined approach.
  • Function with minimal supervision regarding use of time and able to prioritize work assignments.
