Job summary

Rocky Mountain Human Services in Denver, CO is seeking a Service Coordinator to assist in planning and coordinating Early Intervention services for children with developmental disabilities and their families.

You will schedule IFSP meetings, document progress, connect families with providers, and advocate for needed services while maintaining confidentiality and strong organizational skills. A Bachelor's degree or equivalent experience is required, with bilingual abilities preferred.

Qualifications

  • Bachelor’s degree in Early Childhood Education/related field or five years of related experience.
  • Fluent reading, writing, and speaking English.
  • Strong verbal and written communication skills.
  • Excellent organizational skills.

Responsibilities

  • Schedules IFSP meetings and documents progress toward goals.
  • Maintains documentation in EI Colorado Salesforce/Unicorn.
  • Uploads EI documents in the Astro data system.
  • Builds productive relationships with families and service agencies.
  • Assists in coordinating services and supports per agency goals.
  • Completes paperwork to insure continued eligibility for Target Case Management (TCM).
  • Performs duties to established standards ensuring high-quality services.

Position Purpose

The Service Coordinator (SC) assists in planning, coordination and monitoring of Early Intervention services provided to children with developmental disabilities and their families. SC schedule home visits, create individualized family service plans, find early intervention providers, document progress toward goals and assist the family in obtaining necessary services and supports. This position coordinates supports for families utilizing a variety of community resources and programs. They also assist the family through advocacy or mediation in getting what they need from various community providers (i.e., Denver Public Schools).

Essential Duties
  • Schedules Individual Family Services Plan (IFSP) meetings, documents progress toward established goals and assists in obtaining necessary services or supports.
  • Maintains appropriate documentation of plans and progress in developing necessary services or supports.
  • Ensures child specific information is documented and reported timely and accurately to EI Colorado via the data system Salesforce/Unicorn.
  • Ensures child specific Early Intervention documents are uploaded and stored in the Rocky Mountain Human Services data system Astro.
  • Helps Rocky Mountain Human Services implement agency goals and objectives according to the organizations mission and values.
  • Develops and maintains productive and supportive relationships with the families, service agencies, and others involved with the person, as appropriate
  • Maintains the record as required by the Department of Human Services regulations and all funding agencies.
  • Participates in meetings and does any necessary follow-up to department agency assignments.
  • Completes paperwork to insure continued eligibility for Target Case Management (TCM) services as applicable.
  • Performs all job duties to established department standards related to quantity of work while ensuring high-quality services and supports are provided to children and families.

QualificationsMINIMUM QUALIFICATIONS
  • Bachelor’s degree in Early Childhood Education/related field or a minimum of five years related experience
  • Ability to read, write and speak English fluently
  • Excellent verbal and written communication skills
  • Excellent organizational skills
PREFERRED QUALIFICATIONS
  • BILINGUAL - Spanish / English
  • Bachelor’s degree in Early Childhood Education/related field
  • Knowledge of resources, policies, and history of EI systems at local, state, and national levels
  • Experience working with children ages zero to threeTwo years case management experience working with people with developmental disabilities and their families
  • Ability to read, write and speak Spanish fluently
Driving Requirements
  • Valid driver’s license
  • Proof of motor vehicle insurance
  • Personal vehicle in good operating condition for use during work
  • No major violations in the past three years.
  • No more than two moving violations in the past three years
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
