DCBDD provides support to people with developmental disabilities through two core areas: Early Intervention and Service and Support Administration.

The Early Intervention (formerly known as Help Me Grow) program’s goal is to identify and serve children under the age of 3 with developmental delays and disabilities as early as possible. Parents with concerns regarding their child’s development can have their infant or toddler evaluated by an interdisciplinary team at no cost to the family. Children are evaluated for delays in the areas of adaptive, cognitive, communicative, physical, and social-emotional development, using appropriate diagnostic tools.

If a child is determined eligible and has an established unmet need, the child and family will be connected with a Core Team of professionals. The Early Intervention team, comprised of a Service Coordinator, a Developmental Specialist, Occupational Therapist, Physical Therapist, and Speech Language Pathologist will assist the family in creating an Individualized Family Service Plan (IFSP). An IFSP is a family-driven document that is meant to be used as a road map for each individual family’s services and identifies the outcomes that a family wants their child to meet. DCBDD strives to assist families in providing services in the child’s natural environment, such as the home, day care, or other places where the family spends time.

Primary Objective: A Service Coordinator is the key contact for the family as it relates to Early Intervention services and has responsibilities to work on behalf of the family and child through eligibility determination, IFSP development, and service access, provision, and monitoring.

Job Standards

Minimum Qualifications: Associate’s degree in field related to working with infants and toddlers or individuals with developmental disabilities; Bachelor’s degree in related field preferred.

Must be able to obtain and maintain Service Coordinator credentials through the Ohio Professional Registry (OPR), which is maintained by the Ohio Child Care Resource and Referral Association (OCCRRA).

Must possess a State of Ohio Driver's License and acceptable driving record as stipulated by the Board’s insurance carrier.

Physical Requirements: The physical requirements of the position are identified as light work, which may require the lifting of up to twenty-five (25) pounds occasionally.

The physical activity of the position is manual dexterity, talking, hearing, reaching, and navigating your environment.

Ability to operate standard office equipment in the performance of essential functions (e.g. copy machine, fax machine, personal computer, and telephone).

Other Requirements : Successful completion of a criminal records check (BCI and/or FBI) and registry/database check as required by DODD, driving record checks, and DCBDD receiving favorable reference reports.

Schedule may need to be flexible based on DCBDD operational need and/or the individual or families served.

Essential Functions

Serves as the single point of contact for all Part C Early Intervention Services to assist families throughout their EI journey beginning with initial contact through transition at age three (3).

Provides services in the client’s natural environment, most often within the client’s home.

Ensures all evaluations and assessments are completed within a forty-five (45) day timeline from the date of referral; identifies priorities and establishes initial plans that address all family priorities and concerns.

Facilitates the development and implementation of the Individual Family Service Plan (IFSP) that identifies needed EI services and their funding sources within the required timelines.

Coordinates, facilitates, and monitors the timely receipt of services and ongoing provision of services as indicated in the IFSP.

Coordinates and collaborates with medical and other health providers, educational organizations, and other public and private agencies.

Monitors individual and family status following IFSP implementation and revises plan based upon new assessment data.

Provides referrals for additional services, as needed, and coaches families through outcome exploration.

Coordinates transition plans to other programs.

Promptly submits and maintains accurate and complete records and documentation as required by federal/state guidelines, DCY policy and administrative regulations; submits required documentation as identified by DCY and DCBDD record retention policy.

Participates in reflective supervision, per DCY policy, agency EI Administrative meetings, and trainings as required.

Works with community partners to provide quality and effective Early Intervention services.

Schedule may need to be flexible based on needs of the agency and/ or person served.

Regular attendance, as governed by and in accordance with applicable rules, regulations, procedures and laws, is regarded as an essential requirement of this classification.

Adheres to all ORC, OAC, and DCBDD policies and procedures.

Performs other related duties as assigned.
