Responsibilities:

The Service Coordinator’s responsibilities are:

1. Development of the person centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals
2. Coordinate services for Participants in relation to their identified needs
3. Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providers

The Service Coordinator position can be performed remotely and will require travel within Dauphin County.

Education/Experience:

Position Requirements:

• Must have a Bachelor's degree in social work, human services, psychology or other related fields with practicum experience, or in lieu of a Bachelor's degree have at least three (3) or more years of experience in a social service or healthcare related setting.
