Responsibilities

• Consults with medical, ancillary professionals and other team members to provide optimum patient care.
• Demonstrates evaluation skills and interventions, and updates plan of care as appropriate.
• Maintains documentation, patient records, and reports in a timely, compliant manner.
• Uses aides and support staff effectively.
• Communicates with patient/family to provide education and relevant materials.
