Job summary
Monadnock Family Services is seeking a First Episode Psychosis Case Manager to provide comprehensive case management and support for individuals experiencing their first episode of psychosis. You will work within a multidisciplinary team to coordinate treatment, connect clients with community resources, and support recovery and well-being.
Responsibilities include assessing clients, designing individualized care plans, crisis intervention, and advocacy with families.
Qualifications
- Bachelor’s degree in social work, psychology, psychosocial rehabilitation or related human services field.
- Experience: 2+ years in mental health case management; early psychosis intervention is desirable.
Responsibilities
- Assess clients presenting with first episode psychosis and develop individualized care plans.
- Coordinate treatment with multidisciplinary team and families to support recovery.
- Monitor progress and adjust plans; provide crisis intervention when needed.
- Connect clients with housing, vocational training, and social services.
