Job summary

Bright Beginnings, serving Ohio families, seeks a Case Manager Early Intervention Service Coordinator to help families of children from birth to age three access services and rights under federal law. You will coordinate assessments, services on the Individualized Family Service Plan (IFSP), and transition activities within a multidisciplinary team.

The role requires a Bachelor's degree or two years of relevant experience, strong communication, and the ability to maintain confidential family

Qualifications

• Bachelor’s degree in a related field or two years of verified supervised case management experience with young children birth to age five.
• Experience engaging with families, community resources, and education agencies.
• Ability to maintain confidentiality and use sound judgment.

Responsibilities

• Assist families of children from birth to age three to access services and rights under federal law.
• Coordinate developmental evaluations and services on the child’s IFSP.
• Coordinate transition activities and work with a multidisciplinary team.
• Maintain comprehensive records and provide resources to caregivers.
