Job summary

East Central District Health Department/Good Neighbor Community Health Center is seeking a Services Coordinator to assist families with children aged 0-3 with special needs. This position is integral in connecting families to available resources and services, acting as the main contact for families and community providers.

The ideal candidate will have experience in health services, strong organizational skills, and the ability to work with diverse families. The role includes coordinating support through the Early Development Network and maintaining accurate records in compliance with regulations.

Qualifications

• Minimum of two years’ experience in a health or human services field.
• 24 semester hours in behavioral or social sciences.
• Strong skills in communication and organization.

Responsibilities

• Meet with families to review needs and goals.
• Help families access services and resources.
• Coordinate referrals with service providers.
• Assist with Individualized Family Service Plans (IFSPs).
• Schedule and lead IFSP team meetings.
• Support families in advocating for their child.
• Identify service gaps and assist with problem-solving.
• Maintain accurate records and follow guidance.
