Job summary

Lyra Health, Inc. is seeking a full-time Organizational Services Clinical Care Navigator to support organizations and employees, providing expert clinical consultation, CISD coordination, and risk assessment.

You’ll serve as the first clinical contact for HR, ER, and benefits teams, ensuring access to high-quality care. The role requires an MSW with LCSW or equivalent licensure, 2+ years post-licensure, and US residency.

Qualifications

• Master’s degree in clinical social work or equivalent; LCSW or equivalent independent clinical licensure is required.
• Certified Employee Assistance Professional (CEAP) preferred.
• Two years post license experience; EAP/clinical call center experience preferred.
• Significant experience managing high-risk clinical situations (e.g., suicidality).
• Must be a full time resident of the United States; work cannot be completed from outside the United States.

Responsibilities

• Provide clinical consultation to leaders, HR, and ER teams on behavioral health concerns and support strategies.
• Coordinate and facilitate responses to critical incidents, including follow-up care outreach.
• Deliver discreet, high-touch clinical support to executives and VIP employees.
• Respond to calls and emails from HR/ER/benefits leaders with clinical guidance and Lyra resources.
• Reach out to high-risk employees, conduct risk assessments, and navigate to care.
• Collaborate with internal departments to ensure coordination of services and contractual alignment.
• Maintain confidential records in compliance with HIPAA and Lyra protocols.
• Independently manage a caseload of complex, multi-need clients.
