Job summary

Options for Southern Oregon seeks a Wraparound Facilitator/Care Coordinator to work with families and youth, identifying needs, engaging the team, and coordinating services across agencies. The role emphasizes listening, communication, and teamwork to align goals with family priorities.

Responsibilities include organizing meetings, developing care plans, and documenting services for billing, with travel between sites required.

Qualifications

• Bachelor’s degree in human services or three years of applicable experience.
• Completion of Oregon Health Authority approved Wraparound foundational training within 90 days of hire date.
• Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.
• Must pass state-required background and DMV checks; possess a valid OR driver’s license and acceptable driving record.

Responsibilities

• Organizes, schedules and facilitates Child and Family Team meetings.
• Develops, writes and updates Care Coordination plans.
• Documents all services for billing and tracking; completes required documentation.
• Coordinates services offered by multiple agencies.
• Collaborates with families and child-serving agencies to coordinate services.
• Coordinates care and transitions with local treatment providers.
• Provides consultation to agencies and the public about services at all levels.
• Seeks resources for families beyond the mental health system.
• Helps youth and family strengthen their support network.
• Travels between sites and is required to drive.

Education

Bachelor’s (BA/BS) Degree in a human services field

Three years of experience providing services in similar setting

Oregon Health Authority Wraparound training within 90 days

QMHA on MHACBO registry
