Job summary

Qualifications

• Bachelor’s/Master’s/Doctorate in Physical Therapy from an accredited program.
• Current Physical Therapist license/registration.
• Experience in geriatric rehabilitation preferred.
• CPR certification required.
• Ability to lift 50-70 pounds and assist in patient transfers.

Responsibilities

• Perform patient evaluations and develop treatment plans.
• Supervise Physical Therapy Assistants and aides in direct patient care.
• Document evaluations, treatments, progress notes, and discharge summaries.
• Communicate with families, physicians, and interdisciplinary team members.
• Educate patients and families on follow-up care and home programs.
• Collaborate with discharge planners and care teams to optimize outcomes.
