Job summary

Sweetwater Care is seeking a dedicated Physical Therapy Assistant to support rehabilitation from illness or injury in a post-acute care setting in California.

Under the supervision of licensed therapists, you will implement individualized treatment plans, document services for billing, ensure compliance with state and federal guidelines, and coordinate referrals as needed. Excellent patient care and teamwork are essential.

Qualifications

• High School Diploma required
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care to residents to recover and rehabilitate under supervision of licensed therapists.
• Assist in delivering appropriate aspects of quality patient care under supervision of licensed therapists.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Assure that patient care follows the established Plan of Care and organization guidelines, meeting professional standards and legal requirements.
• Under supervision, identify and develop a comprehensive plan of care and implement individualized therapy programs.
• Identify need for and refer to other disciplines to address the patient’s comprehensive needs under supervision of a Physical Therapist.
• Other duties and responsibilities may be assigned at any time.

Education
