Evolve Therapy Services in Gatesville, NC is hiring a Physical Therapist Assistant for an in-house Skilled Nursing Facility to support our therapy team and residents. Under the supervision of a Physical Therapist, you will conduct screenings, participate in treatment planning, administer individualized and group therapy, educate residents and families, and ensure CMS-compliant documentation.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Maintains licensure and certification as required.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing target lists with short-term and long-term goals and treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan (strength, mobility, balance, gait, ROM, modalities).
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents’ response and adjust with PT guidance as needed.
• Assist in scheduling in coordination with the PT and team members.
• Adhere to resident schedules and modify as appropriate for treatment regimens.
• Train residents, families, and staff in equipment use.
• Participate in resident, family, and staff education.
• Maintain sound judgment in evaluation, planning, implementation, and follow-up of programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance activities and CMS-compliant documentation in NetHealth EMR.
