Job summary

Bayshore Nursing & Rehab is seeking a Physical Therapist Assistant (PTA) for a per diem role in Wisconsin. You will work with licensed physical therapists to implement individualized treatment plans and provide patient education to promote independence and mobility.

Candidates must be a graduate of an accredited PTA program with current Wisconsin PTA certification. You will collaborate with the care team and help maintain accurate records in a fast-paced environment.

Qualifications

• Graduation from an accredited Physical Therapist Assistant program.
• Current PTA certification in the state of Wisconsin.

Responsibilities

• Assist physical therapists in the development and implementation of treatment plans.
• Conduct therapy sessions under supervision of licensed physical therapists.
• Provide patient education to promote independence and self-care.
• Maintain accurate records of patient treatment plans, progress, and outcomes.
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
• Participate in patient assessments, progress reports, and care conferences.
• Maintain a clean and safe environment for patients and staff.
