Job summary

Heritage Manor Care in West Hollywood, CA is seeking a passionate Physical Therapy Assistant to join our rehab team. You will provide PTA‑level therapy services under PT supervision to residents in a skilled nursing/assisted living setting.

Requirements include an Associate of Arts in Physical Therapy Assistant, CA licensure/certification as required, CPR certification, and 1 year PTA experience preferred, with 1–2 years in long‑term care preferred.

Qualifications

• Associate of Arts in Physical Therapy Assistant required.
• Licensure or Certification as required by State guidelines; CPR certification.
• 1 year PTA experience preferred; 1–2 years in residential/assisted living or SNF preferred.

Responsibilities

• Provides Therapy services established by the PT in the patient Plan of Care.
• Collaborates with PT regarding changes in treatment or condition.
• Completes documentation including daily notes and progress summaries as needed.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, physicians, and teammates.
• Attends continuing education to keep clinical skills up to date.
• Maintains licensure and equipment in good working order.
