Qualifications

• Active PT license in the state of practice (Arizona).
• Strong clinical assessment and documentation skills.
• Excellent communication with patients, families, and teams.
• Ability to educate patients and families on home programs.
• Ability to work independently and with interdisciplinary teams.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic interventions to improve mobility and function.
• Monitor progress and adjust plans as needed; document outcomes.
• Educate patients, families, and caregivers on plans and home exercises.
• Collaborate with physicians and care teams to coordinate care.
• Maintain accurate clinical documentation per policy and regulatory standards.
• Ensure patient safety and infection prevention throughout care.
• Treat patients across assigned settings and age groups.
• Perform other job-related duties as assigned.
