Job summary

Atrius Health, part of the Optum family, seeks an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our Boston, MA team. You will provide comprehensive, longitudinal care in a primary care setting and collaborate with specialists and staff to coordinate patient care.

Qualifications

• Graduation from an accredited NP or PA program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Mass DPH Controlled Substances Registration or ability to obtain post-hire.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification or professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Performs examinations, diagnoses and treatment in a primary care setting, including physical assessment and treatment; supervises care plans, referrals and communications with specialists as appropriate.
• Oversees monitoring and management of patients with acute and chronic illnesses; acts as care plan author and supervisor; adjusts regimens based on response.
• Partners with patients and families to manage care plans.
• Provides consultation regarding hospitalized patients in the panel.
• Provides direction and support to staff to assure departmental effectiveness and efficiency.
