Qualifications

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
• Active, unrestricted MA NP or PA license.
• National Board Certification.
• Massachusetts DPH CS registration or eligibility.
• DEA licensure/authority or eligibility.
• BLS certification.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of a patient panel.
• Examination, diagnosis and treatment in a primary care setting with care plans.
• Monitor and manage acute/chronic illnesses; author/supervise care plans.
• Collaborate with patients and families to manage care plans.
• Provide guidance to staff to ensure departmental effectiveness.
