Job summary

Creative Solutions in Healthcare - West is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team. You will help residents improve mobility, strength, and quality of life under PT supervision.

Responsibilities include administering prescribed PTA treatments, implementing care plans, documenting progress, and coordinating with the interdisciplinary team. A valid Texas PTA license and SNF experience are preferred; Net Health experience is a plus.

Qualifications

• Must hold active Texas Physical Therapy Assistant license.
• Experience in SNF or long-term care preferred.
• Proficient with Net Health Electronic Documentation System.

Responsibilities

• Administer medically prescribed PTA treatments to improve mobility.
• Provide PT services under supervision of a PT.
• Respond to requests by relaying information to the PT.
• Implement and modify care plans under PT supervision.
• Measure and record motor function and strength.
• Document thoroughly per policy and regulations.
• Attend required Rehab Director meetings.
