Job summary

Advanced Health Care of St. George in Vermont seeks a Physical Therapist or Physical Therapy Assistant to deliver high-quality rehabilitation services for residents, focusing on mobility, strength, and independence.

You will perform assessments, create individualized plans, and provide one-on-one therapy in a supportive setting. Responsibilities include documenting progress, collaborating with physicians, nurses, and other therapists, and educating residents and families on home exercise

Qualifications

• Current state licensure as a Physical Therapist or Physical Therapist Assistant.
• 1 year of clinical experience in skilled nursing facility or similar healthcare setting preferred; new grads welcome.
• Strong clinical reasoning and decision-making skills, with proficiency in evaluating and treating a diverse range of conditions and diagnoses.
• Ability to build rapport with residents through a compassionate and caring demeanor.
• Excellent interpersonal and communication skills, with the ability to collaborate effectively with residents, families, and team members.
• Commitment to providing high-quality, resident-centered care with empathy and compassion.

Responsibilities

• Evaluate Residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.
• Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.
• Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.
• Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.
• Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.
• Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologist, to coordinate care and ensure continuity of services.
• Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.
• Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.
• Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices in rehabilitation.
• Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
• Perform all other duties assigned.
