Job summary

Qualifications

• Current state licensure or eligibility for licensure as a Physical Therapist.
• Minimum of 1 year of clinical experience in a skilled nursing facility or similar healthcare setting.
• Strong clinical reasoning and decision-making skills.
• Ability to build rapport with residents through a compassionate and caring demeanor.
• Excellent interpersonal and communication skills, with the ability to collaborate with residents, families, and team members.

Responsibilities

• Evaluate residents' physical abilities and rehabilitation needs through comprehensive assessments.
• Develop personalized treatment plans based on assessment findings and resident goals.
• Implement therapeutic interventions to address impairments and promote safe mobility.
• Provide one-on-one physical therapy sessions to residents.
• Monitor resident progress and adjust treatment plans to optimize outcomes.
• Collaborate with physicians, nurses, occupational therapists, and speech-language pathologists to coordinate care.
• Educate residents and families on exercise programs and home regimens.
• Document all PT services, including evaluations and progress notes, per facility policy.
