Hackensack Meridian Health is seeking a Physical Therapist to evaluate and implement goal-oriented therapy plans, restore function, and prevent disability. The role requires certification and emphasis on safe, effective care and collaborative teamwork.

You will document progress, discharge planning, and educate residents and families while maintaining high standards of care within a supportive environment.

Qualifications

• Graduate of an APTA accredited program or equivalent.
• Licenses: Physical Therapist License and BLS certification.
• Excellent written and verbal communication skills.
• Proficient computer skills (Microsoft Office/Google Suite).

Responsabilités

• Evaluates, plans and implements a goal oriented Physical Therapy treatment program.
• Discharge planning established at initial evaluation and updated during course of treatment.
• Documents progress notes, reassessments and discharge summaries.
• Educates residents and families about treatment plans and device use.
• Maintains safety and proper body mechanics during all therapies.
• Coordinates with other care professionals for optimal resident outcomes.
• Contributes to plan of care and goals aligned with resident needs.
• Adheres to departmental policies and quality standards.
