Job summary

Recover-Care Healthcare is seeking a Physical Therapist to develop and manage individualized therapy programs for patients in our facility. You will assess, plan, implement, and educate to maximize functional progress while collaborating with physicians and ancillary staff.

The role requires a PT license and a commitment to high-quality patient care, documentation, and continuous professional development. Prior SNF experience is a plus, with on-site duties and patient interaction.

Qualifications

• Graduate degree in Physical Therapy (DPT/BPT) from an accredited program.
• Current license in good standing with the state Board of Physical Therapy Examiners.
• Proficiency with standard PT assessment and documentation practices.

Responsibilities

• Provides direct patient care and evaluates outcomes to inform treatment plans.
• Designs and implements an individualized plan of care with patient involvement.
• Delivers therapy within defined standards of care and updates plans regularly.
• Coordinates with health professionals and staff to achieve goals.
• Maintains documentation, goals, and prescriptions with accuracy.
• Adheres to safety, infection control, and departmental policies.
• Engages in ongoing professional development and literature updates.
