Job summary

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our growing team in SNF settings. The PT will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high-quality outcomes.

The role includes developing plans of care with measurable goals, educating patients on progress and probable outcomes, supervising Physical Therapist Assistants, and maintaining complete documentation.

Qualifications

• Experience evaluating and treating SNF patients.
• Experience supervising Physical Therapist Assistants (PTAs).
• Strong documentation and record-keeping skills.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Develop plans of care and set measurable functional goals.
• Provide patient education on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Complete timely and accurate documentation including evaluations and progress notes.
• Collaborate with facility staff to ensure excellent patient care.
