Job summary

Northwoods Rehab is seeking a Physical Therapist to provide and direct patient-centered PT services. The role includes assessment, treatment planning, documentation, and coordination with the care team under physician orders.

Ongoing education and professional development are emphasized. The candidate must hold a Doctor of Physical Therapy or equivalent, be eligible for licensure, and be committed to delivering high-quality patient care in a supportive setting.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.

Responsibilities

• Provides and directs physical therapy services to patients, including assessment, treatment, program planning and implementation, related documentation and communication under physician’s orders.
• Provides physical therapy services to patients and initiates referrals when appropriate.
• Conducts screenings of patients to determine need for intervention and/or treatment.
• Evaluates patients to obtain data for treatment planning and implementation.
• Adapts treatment to meet individual patient needs and documents results.
• Coordinates treatment plan with patient, family and other team members.
