Northwell Health seeks a Physical Therapist to evaluate and treat patients with disabilities, injuries, or diseases, using prescribed physical therapies and maintaining complete medical records.

You will administer care on site, perform thorough evaluations, design individualized treatment plans, monitor progress, and adjust as needed. The role requires NYS licensure and 3+ years of relevant experience; outpatient practice may involve limited permits.

Formación

• Doctoral Degree in Physical Therapy is required.
• Current License to practice as a Physical Therapist in New York State is required.
• Limited Permit to practice in New York State is allowed for outpatient settings.
• Eligible for NYS 90-day post-doctoral graduation practice exemption with conditions.

Responsabilidades

• Administers direct patient care per physician prescription.
• Uses evaluation techniques with efficiency and thoroughness.
• Designs a treatment program to achieve established goals.
• Monitors patient tolerance/status and adapts treatment as needed.
• Responds to department emergency procedures for patient safety.
• Submits required documentation per department policies.
• Participates in quality improvement chart reviews.
• Attends family meetings with inter-disciplinary staff.
• Performs related duties; all essential under ADA.
