Job summary

Physical Rehabilitation Network, LLC. is seeking a licensed Physical Therapist to provide exceptional outpatient rehabilitation in Chandler, AZ.

You will assess, plan, and implement personalized therapies to improve mobility, reduce pain, and restore function within a collaborative care team. You will educate patients and families, document progress, and stay current with therapy techniques and regulatory requirements.

Qualifications

• Must hold valid state licensure as a Physical Therapist.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• CPR certification and other state regulatory requirements as applicable.
• Strong clinical skills in orthopedic, neurological, or geriatric physical therapy (preferred).
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a team.
• Proficient in documentation and record-keeping.

Responsibilities

• Conduct comprehensive assessments to determine patients' functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
• Implement and administer therapeutic exercises, manual therapy, and other interventions to improve mobility, strength, flexibility, and function.
• Maintain accurate and organized documentation of patient progress and treatment outcomes.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Educate patients and families about therapy procedures, home exercises, and injury prevention techniques.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain timely documentation of patient evaluations, progress notes, and discharge summaries.
• Stay updated on advancements in physical therapy techniques and best practices.
• Meet patient needs by following up on Plan of Care and addressing noncompliance where applicable.
• Delegate and utilize PTA support when appropriate.
