Job summary

Physical Rehabilitation Network, LLC. is seeking a licensed Physical Therapist to deliver exceptional rehabilitation services in Surprise, AZ. You will assess patients, design tailored treatment plans, and guide exercises and modalities aimed at restoring mobility and reducing pain.

Engage with a multidisciplinary team, document progress meticulously, and educate patients and families on home programs while pursuing ongoing professional development and best practices.

Qualifications

• Valid state licensure as a Physical Therapist.
• DPT or Master’s degree in Physical Therapy from an accredited program.
• CPR certification and state regulatory requirements.
• Experience in orthopedic, neurological, or geriatric PT preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and in a team.

Responsibilities

• Conduct comprehensive assessments of patients' functional abilities and goals.
• Develop personalized treatment plans using various therapies and modalities.
• Administer therapeutic exercises, manual therapy, and other interventions.
• Maintain accurate documentation of progress and outcomes.
• Monitor progress and adjust plans to achieve optimal results.
• Educate patients and families on therapy procedures and home programs.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Ensure timely documentation of evaluations, progress notes, and discharges.
• Stay updated on PT advancements and apply them appropriately.
• Support caseload with nonclinical staff and delegate to PTA when applicable.
