Job summary

Responsibilities

• Evaluate patients and interpret examination findings to determine dysfunction and plan care.
• Establish plan of care to achieve goals considering age and medical history.
• Administer modalities and prescribed exercise programs.
• Supervise PT assistants and aides as appropriate.
• Coordinate discharge planning with care team and participate in rounds.
• Provide patient and family education and ensure documentation per policy.
• Maintain compliance with Kaiser Permanente policies and safety guidelines.
• Cover diverse patient populations per local center needs.
