Job summary

Kaiser Permanente is seeking a physical therapist to provide comprehensive management of patients, reporting to the Director, Assistant Director and/or Clinical Supervisor.

The role emphasizes evaluating patients, developing individualized plans of care, administering modalities and exercise programs, and coordinating discharge planning with the care team while maintaining clinical documentation and compliance with policies.

Qualifications

• PT degree required (Bachelors, Masters or Doctorate).
• Licensed Physical Therapist in California at hire or eligible at hire; license within 4 months of hire.
• National Provider Identifier required at hire.
• Basic Life Support (BLS) required at hire (American Heart Association).

Responsibilities

• Evaluates patients and interprets examination findings to determine dysfunction and restore mobility.
• Establishes plan of care to achieve goals based on age and medical history.
• Administers modalities and/or exercise programs.
• Documents therapy services per state/federal requirements and policies.
• Coordinates discharge planning with other health care personnel.
• Supervises PT assistants and aides when appropriate.
