Job summary

Collaboration with the Director of Rehabilitation Services and care team is essential to support recovery. Responsibilities include patient counseling, maintaining records, and providing discharge summaries in line with policy.

Qualifications

• Active state licensure as a physical therapist.
• Experience diagnosing movement dysfunction and developing treatment plans.
• Ability to supervise PT assistants and rehab aides.

Responsibilities

• Consult with patients to learn about their condition and review medical history.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans and teach therapeutic techniques.
• Maintain patient records and document progress and discharge summaries.
• Provide counseling to patients and families and communicate with the care team.
