Centra Healthcare Solutions Inc. is seeking an experienced or newly graduated Physical Therapist (PT) for a direct hire/permanent role in a SNF/LTC setting with a daytime schedule.

The role requires graduation from an accredited PT program, current state PT licensure (or in process) and CPR certification. You will develop care plans, provide PT services per physician orders, and collaborate with the care team to achieve patient goals.

Qualifications

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or in the process of licensure.
• Current CPR certification.

Responsabilités

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
