Job summary

Centra Healthcare Solutions is seeking a Physical Therapist / PT Manager for an outpatient facility. The role involves direct patient care, assessing rehabilitation needs, and developing treatment plans under physician direction.

Ideal candidates have a PT license, graduated from an accredited program, CPR certification, and are prepared for both experienced and new graduates to join a growing team.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Current state license as a Registered Physical Therapist (PT).
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
